Please ask your provider for a referral to our office.  We do not accept self-referrals. 

Please have your provider send progress notes, relevant labs, cultures, imaging etc. prior to appointment.

Please have an updated medication list ready for your appointment. 

  If you need to cancel or reschedule your appointment,

please call (949) 515-3590 as soon as possible. 

• We kindly request at least 24 hours’ notice so we can offer the appointment time to another patient in need.
• Missed appointments without appropriate prior notice of cancelation, may result in a fee.
• If you are unable to attend in person, we may be able to convert your visit to a telemedicine appointment—please contact us as early as possible to make this change.
